The search for the DentaQuest 2021 Health Equity Heroes has officially begun. Nominees can come from any sector, including dental care, healthcare, family support, legal aid, schools and community programs.
The DentaQuest Health Equity Hero awards recognize individuals and organizations behind extraordinary work that champions equitable oral care and healthcare.
Fill out and submit this form before April 30. DentaQuest will announce the winners in June.
DentaQuest will choose 12 Health Equity Heroes for 2021. Each will receive:
- A commemorative plaque, highlighting their accomplishment
- $5,000 donation to a nonprofit — the Hero gets to choose — that is committed to promoting health equity
- Recognition in DentaQuest outreach and communication materials throughout the year
